The following form is for faculty and staff to report any type of incident to the Surry Community College Campus Police.
Report an Incident (Word Document) - Requires faculty/staff login to access.
To use this form you may either:
- Right click on the link, select "Save Target As..." and save the file to your local computer.
- Type in your information.
- Save your changes.
- Attach the document to an email to Chief Patrick and/or Sergeant Shropshire.
OR
- Left-click on the link to open the Word document.
- Print the document.
- Fill in your information.
- Forward the document to Campus Police via Campus mail.
