Report An Incident

The following form is for faculty and staff to report any type of incident to the Surry Community College Campus Police.


Report an Incident (Word Document) - Requires faculty/staff login to access.

Don't have Word on your PC? Download Word Viewer 2003 to view, print, and copy Word documents even if you don't have Word installed.

To use this form you may either:

  1. Right click on the link, select "Save Target As..." and save the file to your local computer.
  2. Type in your information.
  3. Save your changes.
  4. Attach the document to an email to Chief Patrick and/or Sergeant Shropshire.

OR

  1. Left-click on the link to open the Word document.
  2. Print the document.
  3. Fill in your information.
  4. Forward the document to Campus Police via Campus mail.
