If you are interested in participating in the cohort, the first step is to complete the cohort application and return it to the Office of Distance Education.

To complete the Cohort Application follow the steps Listed below

1- Save the Word XP Document to your hard drive.

Directions on how to save the cohort applications

1. - Using your mouse point to the cohort application link above. 
2. - Click the link using your right mouse button.
3. - A menu appears with an option to Save target as... Choose this option.
4. - Save the file to your hard  drive. 

2- Open the cohort application in Word XP and complete the form. 

3- Save the completed application to your hard drive.

4- Email the completed cohort application to Dr. Candace Holder, Director of Distance Education at holderc@surry.edu.
